PCN Pharmacy Technician

The Pharmacy Technician will work under the supervision of the clinical pharmacist to help GP practices pro-actively keep patients safe, well and out of hospital, as well as helping to reduce the demands on general practice. PCN Care Co-ordinator Administrator

NCGPA are looking for an enthusiastic, confident, flexible, and highly organised person to work within the Primary Care Network (PCN). This is a new and multifaceted role supporting the PCN with the delivery of the Direct Enhanced Service Contract (PCN DES), through the provision of patient care co-ordination, and administrative support for the PCN, Clinical Director, Network Manager and PCN workforce. PCN Social Prescribing Link Worker

Social prescribing is fundamental to the vision of NHS England and NHS Improvement in delivering personalised care to millions of people through the NHS Long Term Plan. Social prescribing connects people to practical and emotional community support, through social prescribing link workers, who are based in GP practices and take referrals from all local agencies.

It helps people get more control over their healthcare, to manage their needs and in a way that suits them. It can especially help people who:

Prescription Clerk at Bilborough Medical Centre

The Prescription Clerk is responsible for the processing of repeat prescription requests from patients.
These can be received by email, sent via the clinical on line system or by the patient bringing a request form into the practice. The Prescription Clerk will be expected to process all relevant prescriptions and re-issue them for authorisation by Bilborough Medical Centre’s GPs, Clinical Pharmacists and Prescribing Nurses, in accordance to the Dispensing Guidelines.

They will be responsible for ensuring that any prescription request that requires a medication review or needs clinical input is tasked to the Clinical Pharmacist for action, to ensure continuity of care. They will be responsible for ensuring monitoring blood tests have been done prior to issuing certain medications and that medications are not being ordered outside of the issue duration period for patient safety.
