Health and Wellbeing Coach (Nottingham City East PCN)

This role will sit within the broader PCN primary care team as an extension of the wider provision available.

It requires the individual to be able to assess patient needs, helping patients to gain and use knowledge, skills and confidence to become active participants in their care so that they can reach their self-identified health and wellbeing goals. The role will include being responsible for delivering group and one on one sessions, face to face or remotely, on topics which may include topics such as healthy eating, physical activity and health and wellbeing to meet personalised care plan goals; engaging with patients to support them in taking an active role in their health; providing advice and support for both patients and primary care colleagues.

Safeguarding Co-Ordinator

We are looking for a Safeguarding Care Co-ordinator to work across the Nottingham City East Primary Care Network (PCN) to support our Safeguarding Lead GPs and the established PCN Safeguarding Co-ordinator in protecting our most vulnerable patients. Nottingham City East Primary Care Network (PCN) is a network of seven practices with a list size of around 66,350 patients, with 62% of the population living in areas defined as the most deprived 20% in England.

Podiatrist

This post requires a podiatrist who is highly specialised and has considerable knowledge, clinical skills, and experience at a senior level, in the management of patients with lower limb pathology e.g. musculoskeletal, dermatological, neurological and vascular.

It is based within the Nottingham City East Primary Care Network (PCN), to work alongside the multi-disciplinary PCN practice teams to effectively plan and implement a high-quality Podiatrist service as part of the broader PCN Primary Care Team.

PCN Care Co-ordinator Administrators

NCGPA are looking for an enthusiastic, confident, flexible, and highly organised person to work within the Primary Care Network (PCN) Team. This is a new and multifaceted role supporting the PCN with the delivery of the Direct Enhanced Service Contract (PCN DES), through the provision of patient care co-ordination, and administrative support for the PCN, Clinical Director, Network Manager and PCN workforce. This is a patient facing role within front line NHS services.
